We haven't made any of the answers mandatory, so you don't need to include personal information if you don't want to. 

However, we do ask that where possible, you share certain details if you feel comfortable, particularly an email address. This isn't because we will be publishing it or passing it on, it's so we can verify as many responders as possible and be confident in the results we get.
